Welcome New Patients

During your first appointment, our doctors will spend forty-five minutes to an hour performing a thorough examination to determine the scope of your problems. This includes a one-on-one discussion about your health history, current concerns, and any supplements or medications you are currently taking. You are encouraged to ask questions. Once your examination is complete, our doctors will also perform a chiropractic adjustment if deemed appropriate. Please wear comfortable clothing that allows for easy movement. This is good to keep in mind for all future visits as well.

We ask that you bring X-rays, MRIs, and other imaging taken within the last three years to your initial appointment. If you do not have any current imaging but our doctors feel it’s needed to better understand the nature of your issues, you may be referred to a local imaging center.

Each patient is required to complete a New Patient Health History form. When you book your initial appointment online, you will receive a welcome email with online forms to fill out. You can submit the forms through our online portal along with uploading any insurance cards or documents that you wish for the doctor to have. If you opt out of the online portal, then please plan to arrive at our office 15 to 20 minutes before your initial visit to fill out the necessary paperwork. 

Additional forms are also required and include: Informed Consent, Financial Policies, and the HIPAA Signature Form. Please read through the HIPAA Privacy Notice 2016 then print and sign the additional patient forms.

Please remember to bring a valid driver’s license or photo ID, along with your insurance card, so we can make a copy for your file.

At Front Street Chiropractic we keep patient records for seven years following a patient’s last date of service. We will destroy all records for patients whose last date of service was more than seven years ago. The destruction of all files is confidential and HIPAA compliant. Please call 303-604-2987 if you do not want your information destroyed.
